Cells have engines

Inside almost every cell are hundreds of mitochondria. Their job is to take food and oxygen and turn it into ATP — the only kind of energy your body can actually spend. Muscles, heart, and brain are packed with them, which is why those are the first places to complain when mitochondria are struggling.

Making energy is a little like running an engine: you get power, and you get exhaust. The exhaust here is free radicals — reactive molecules that nick the engine's own parts. Over years, that wear is a big part of why energy drops with age.

What SS-31 does

SS-31 (also written elamipretide) is unusual: it's built to travel into mitochondria and stick to their inner wall, where it binds a fat called cardiolipin. Cardiolipin is the scaffolding the energy machinery sits on. When it's damaged, the assembly line leaks and makes even more free radicals. SS-31 is studied for stabilizing that scaffolding — less leaking, less exhaust, engines that keep running properly.

What MOTS-c does

Here's the surprising part: mitochondria have their own small set of DNA, separate from the DNA in the cell's nucleus. MOTS-c is a peptide written in that mitochondrial DNA — the engine sending a memo to headquarters. Research links it to AMPK, the sensor that notices when energy is low and switches cells into efficient, fat-burning, build-more-engines mode. It's the same switch exercise and fasting flip, which is why MOTS-c is often described in papers as an exercise-mimetic.

Why together

Picture a delivery company with a beat-up fleet. SS-31 is the mechanic — fixing leaks so the trucks you own stop breaking down. MOTS-c is the manager — deciding to run more efficient routes and buy more trucks. Hire only the mechanic and your fleet is healthy but small. Hire only the manager and you buy new trucks while the old ones burn in the lot.

Because one is protective and the other is a growth signal, they're studied as sequential rather than interchangeable. Reduce damage first, then ask the system to expand — that's the logic behind pairing them.
